When an engine needs replacing
Engine replacement is due when internal damage is so extensive that component-level repair is no longer economically sensible. Typical situations: crankshaft bearing failure from oil loss, a major head gasket failure combined with a warped cylinder head, a snapped timing belt on an interference engine, or over-aggressive tuning that has broken the pistons. An engine swap always starts with comparing repair options: is component-level repair worthwhile, is a remanufactured engine suitable, or is the car's market value high enough to justify a brand-new engine. The most common choice is a remanufactured engine — it's usually 50 % cheaper than new and has a longer service life than used.
How an engine is replaced
Work starts with removing the old engine. All wiring, fuel lines, coolant hoses, exhaust, gearbox connections and engine mounts are marked and carefully disconnected. The engine is lifted out with an engine hoist — usually from above, but on some models (Volvo, certain BMWs) from below. The new engine is installed and every connection is made per manufacturer specification. Oils and fluids are filled, systems are bled and the engine is started for the first time. In the first 500 km of break-in, heavy loads are avoided and oil pressure, coolant temperature and any leaks are monitored. Finally the engine is coded to the car via the diagnostic tool and a long test drive is carried out.

Why do engines fail completely?
Complete engine destruction almost always comes from one of: oil loss (a leak the driver didn't notice mid-drive), overheating (a cooling system failure the driver didn't notice), snapped timing belt (unexpected failure of a worn belt), or burn-through from an internal fault (worn piston rings causing oil burning and carbon build-up). In all these cases the engine ECU's protection mechanisms don't have time to shut the engine down before internal damage becomes unrecoverable.
